Continuous Quality Improvement (CQI)

Continuous Quality Improvement (CQI) with primary care practices has focused on increasing well child visit rates and improving the experience for families.  Working with individual practices, KHC staff work to obtain input from multiple stakeholders (clinicians, nursing/medical assistant staff, front office staff, patient families and more), and then collaboratively brainstorm goals to be attained and ways to improve.  Using a broad array of tools that cover the gamut of both warm-touch and tech-based options, KHC staff works in conjunction with Medicaid health plans and primary care practices to address barriers.


Programs

FitKids 360

FitKids 360 is an 8-week childhood obesity intervention program that educates children and their families learn about nutrition, physical activity and behavioral health topics.

Throughout the eight weeks, children and their families learn about nutrition, physical activity and behavioral health topics.

In-Home Asthma Care

In-home Asthma care has been proven to reduce emergency and unscheduled doctor visits and improve quality of life for those struggling with asthma. 

The 6 months Asthma MATCH program works to improve a patient’s asthma knowledge, their asthma control, and their quality of life.

LENA Start™

LENA Start is a language enrichment program that helps families create a language-rich environment, which improves school readiness. 

During a 10-week program, families participating in LENA Start receive resources, education and are able to track their progress.
