KHC strives to improve quality, access and health outcomes for children eligible for Medicaid by promoting the Medical Home Model.

Overview of KHC

Kids’ Health Connections (KHC) is a not-for-profit organization located in Detroit, Michigan.

At KHC, we have a culturally diverse staff that works with primary care physicians, health plans, parents, and community agencies in a collective effort to improve the health of children throughout Oakland, Macomb and Wayne Counties.

KHC helps parents and their children get the most out of their relationship with their family physician by bridging gaps and identifying viable health care solutions. KHC provides health education to families, helps doctors improve the quality of their care, and acts as a voice for issues affecting children’s health.

The KHC team works with doctors who refer families for a variety of reasons. These include helping families understand how to manage their child’s asthma through in-home asthma education, improve their lifestyle choices through our Fit Kids 360 program to help combat childhood obesity and the importance of staying up to date on their child’s immunizations.
